How to add a Header Image to a Submission Form

Here's how to add a header image to the top of your form

Adding a logo at the top of a submission form adds a bit of personalisation, colour and legitimacy.

How to Add

  • Navigate to Settings on the platform

  • Click on Projects to see all your active types.

  • Select the Project you want to setup a form for and click Submission Form

  • Select the Appearance option and select Header Image

Add the option to "Change" and select a file from your device and save that in place (PNG or JPEG). This image will then display at the top of the Submission Form. Each Submission Form can have a different image if required.

Header Dimensions

For the main header image that appears at the top of a form:

Recommended ratio: 3:1
Example size: 1800px Γ— 600px

Why this works:

  • It creates a clean banner-style layout

  • It avoids taking up too much vertical space

  • It keeps your branding visible without overwhelming the page
